Please do not send personal health information through this website. Our contact and feedback forms are for general enquiries and scheduling only. Anything about your specific care should be discussed by phone on (718) 714-6995 or in person at the office.
Information you give us
You can use most of this website without giving us any information at all. We only receive what you choose to send us:
- Our contact form asks for your name, email address and phone number, and lets you tell us which service you are interested in, the day and time that suit you best, and anything else you would like us to know. We use this to reply to you and to arrange an appointment.
- Our feedback form asks for your name, email address, a star rating and your comments. This form opens a message in your own email program — nothing is sent anywhere until you send that email yourself, and it goes directly to the practice.
- Calling or emailing us gives us whatever you choose to share in that call or message.
We use this information to respond to you, to schedule and manage appointments, and to keep a record of our correspondence. We do not sell it, rent it, or share it with anyone for their own marketing.
Information collected automatically
Like most websites, this site uses analytics and advertising tools that collect information about how the site is used. These place cookies or similar identifiers in your browser and may record your approximate location, device and browser type, the pages you visit, and how you arrived here.
- Google Analytics 4 — tells us which pages people read and how they move through the site, so we can improve it.
